Covered when ALL of the following are met:
ALL of the following
- Service is an outpatient physical therapy, occupational therapy, or speech therapy service (including evaluations).
- Member is an Anthem HealthKeepers Plus member.
- Service is delivered via telehealth (telemedicine) on or after April 28, 2020 per DMAS guidance during the COVID-19 pandemic.
- Providers follow current authorization requirements and applicable reimbursement and medical policies that would otherwise normally apply.
