Coverage criteria and documentation requirements

Reimbursement is allowed when psychotherapy treats psychiatric and/or substance use disorders and documentation and authorization requirements are met.

Covered when ALL of the following are met:

Service type: Psychotherapy services for psychiatric and/or substance abuse disorders provided through scheduled therapeutic appointments between an approved provider and the patient
Approved providers: Psychiatrists (MDs), Psychologists (PhDs), Nurse Practitioners/Clinical Nurse Specialists (APRNs/NPs) with specialized psychiatric training or experience, Physician Assistants (PAs), and other state‑licensed or authorized mental health providers

Billing and legal requirements

Services must meet authorization and medical necessity guidelines appropriate to the procedure and diagnosis and to the member's state of residence
Use industry‑standard CPT/HCPCS/revenue codes; codes must be supported in the medical record and compliant coding/billing guidelines must be followed
Policy is subject to provider, state, federal, or CMS mandates and member benefit terms; mandates may supersede this policy
If coding/billing guidelines are not followed, Anthem may reject or deny the claim and may recover or recoup claim payment

Authorization and Provider Responsibilities

Prior Authorization

Authorization, medical necessity, and documentation obligations

Services must meet authorization and medical necessity guidelines appropriate to the procedure and diagnosis and to the member's state of residence. Providers must follow proper billing and submission guidelines, use industry-standard CPT/HCPCS/revenue codes, and ensure that billed services are fully supported in the medical record and/or office notes. Failure to follow coding/billing guidelines or reimbursement policies may result in claim rejection or denial and recovery/recoupment of payment.

Obtain any required prior authorization and document medical necessity tied to the procedure and diagnosis.
Use industry-standard, compliant codes (CPT, HCPCS, revenue) on all claim submissions and ensure documentation supports billed codes.
Ensure records are sufficient to support reimbursement per policy and state/provider contractual mandates to avoid denials or recoupment.

DefinitionAn urgent assessment and history of a crisis state including a mental status exam and disposition; treatment includes psychotherapy, mobilization of resources to defuse the crisis and restore safety, and implementation of psychotherapeutic interventions to minimize potential psychological trauma.

Typical presentationLife‑threatening or complex problems requiring immediate attention for a patient in high distress.
