Pharmaceutical waste from single-dose vials (SDVs) may be eligible for reimbursement when the unused portion is reported on a separate claim line with modifier JW and the administered dose is reported on its own line, in accordance with HCPCS billing units.

The provider must report the HCPCS code for the drug/biologic on one line with the actual units administered and report the amount discarded on a separate line of the same claim with modifier JW appended when the actual dose administered from an SDV exceeds the HCPCS billing unit.

Zero or no separate waste report

Append JZ modifier when the actual dose administered from an SDV results in zero waste (no discarded amount).
