Reimbursement and Coverage Criteria
Reimbursement criteria for Modifier 59 and X{EPSU}
Reimbursement allowed when modifier use appropriately indicates a distinct/independent service and is supported by coding rules and documentation; certain situations and code pairs are nonreimbursable.
All of the following must be met:
Modifier and coding framework
- Procedure or service billed with Modifier 59 or an X modifier (XE, XP, XS, XU) to indicate the service is distinct or independent from other services performed the same day by the same provider.
- Modifier use aligns with CMS NCCI Procedure-to-Procedure (PTP) edit guidelines (NCCI Column 1/Column 2); Modifiers 59 or X{EPSU} may be appended to the paid or denied code.
- Modifier 59 is used only if no more descriptive X{EPSU} modifier is available and Modifier 59 is not appended to the same claim line item as an X{EPSU} modifier.
