When Distinct Procedural Service Modifiers Are Reimbursed

Reimbursement criteria for distinct procedural service modifiers

Anthem will reimburse distinct procedural services when billed with Modifier 59 or X{EPSU} consistent with NCCI PTP edit guidance, except in specified nonreimbursable scenarios and when state- or contract-specific exceptions apply.

Follow CMS NCCI Procedure-to-Procedure (PTP) edit guidelines for Column 1/Column 2 code relationships.

ALL of the following

Use XE/XP/XS/XU when a more descriptive X modifier is available.
Use Modifier 59 only if no more descriptive X{EPSU} modifier is available.
Do not append Modifier 59 to the same claim line as an X{EPSU} modifier.

Reimbursable per NCCI: Modifiers 59 or X{EPSU} may be appended to the paid or denied code when appropriate.

Claims submitted with Modifier 59 or X{EPSU} are subject to post-payment review; providers may be required to submit supporting documentation and medical records; insufficient or missing documentation may result in denial or recovery/recoupment of payment.
