Anthem allows reimbursement of duplicate or subsequent services on the same date of service only when appropriately documented and billed with required modifiers or additional units; otherwise lines may be denied or reviewed.

Duplicate or subsequent same-day services must be billed with an appropriate modifier or additional units to be reimbursed; otherwise the claim line may be denied or recouped.

Claims billed with suspected duplicate or subsequent services will be reviewed; services determined to be duplicate or subsequent will be denied without the appropriate modifier(s).

When an individual service is unbundled from a more comprehensive service and billed on the same date of service

If billed on the same claim, the claim line for the individual service will be denied through code editing.
If billed on separate claims, the claim will be reviewed.

- Modifier 25: Significant, separately identifiable Evaluation and Management service by the same physician on the same day of the procedure or other service.
- Distinct Procedural Services: Modifiers 59, XE, XP, XS, XU.
- Repeat or related modifiers that may indicate distinct or repeat services: 76, 77, 62, 66, 80, 81, 82, 91, AS, GG, GH.

Claims for duplicate or subsequent services provided on the same date of service must be billed with an appropriate modifier or with additional units; otherwise the claim may be denied or recouped. Anthem will review claims billed with suspected duplicate or subsequent services and may deny services determined to be duplicate or subsequent when the appropriate modifier(s) are not present.

Include an appropriate modifier (see Related Coding section) or additional units when billing repeated or subsequent services on the same DOS.
Be aware that Anthem will review suspected duplicate/subsequent claims and deny lines lacking the required modifier(s).

Duplicate Services — definition

Definite duplicate criteriaA service is a definite duplicate if some or all of the following elements on the claim match: member; date of service; charge amount; provider of service/rendering provider; type of service (based on procedure or revenue codes).

Suspected duplicateA service is a suspected duplicate if the member and procedure code on the claim match.

Scope noteDefinition used to determine when identical or overlapping claim lines represent duplicate billing subject to denial or review.

Subsequent service — definition

DefinitionA subsequent service is a medically necessary service performed or provided for the same member more than once on the same date of service.
