Coverage Criteria for Veneer Restorations

To qualify for veneer restorations, ALL of the following must be met:

Main criteria: Placement of veneers must be necessary, appropriate and meet generally accepted standards of care.

Main criteria: Must have a reasonable expectation for success.

Placement of veneers is not indicated solely for surface discoloration from non-structural causes. Examples where veneer placement typically does not meet criteria include tooth staining due to prior root canal therapy or drug-related staining, treatment of uneven or misaligned anterior teeth, and closure of diastemas (gaps). Veneers also are not appropriate simply to alter the shape of congenitally malformed teeth when the intervention is primarily cosmetic rather than restorative. Decisions should follow the plan’s definition of generally accepted standards of dental practice and provider documentation requirements when a functional restoration is being claimed.

Veneer placement for purely cosmetic reasons is explicitly excluded and may be denied. This includes use of veneers to correct congenital or developmental problems or to correct tooth size discrepancies when the procedure is performed for appearance rather than restoration of significant tooth structure. Where less invasive restorative alternatives (for example, minor edge repairs or other restorative treatments) would appropriately address the defect, veneers are not considered medically necessary. Coverage depends on contract language and the cosmetic dentistry exclusion; providers should expect review against contractual exclusions and generally accepted standards of care.

Cosmetic or Developmental Indications Are Not Covered — Denial Risk

Veneer placement performed solely for cosmetic purposes, for correction of congenital or developmental problems, or to correct tooth size discrepancies is excluded and presents a denial risk.

Cosmetic-only indications are not considered contractually necessary.
Correction of congenital/developmental anomalies or tooth size discrepancies using veneers is not covered.

Documentation Required

Documentation Requirement — Degree of Tooth Breakdown

Provider must document the degree of tooth breakdown and clinical justification for veneer placement.

Document significant loss of tooth structure and/or involvement of one or both incisal angles or a canine cusp tip.
Provide clinical chart notes and photographic documentation upon request to support the extent of tooth breakdown.
Include documentation of periodontal status and history of definitive periodontal treatment when relevant.

A current (within 12 months) pretreatment periapical radiograph showing the apex is required to evaluate pulpal and periapical status prior to veneer placement.

Radiograph must be no older than 12 months.
Radiograph should demonstrate lack of pulpal and/or periapical pathosis prior to authorization or treatment planning.

Background

Veneers are a conservative restorative option intended to restore function when anterior teeth have sustained significant loss of tooth structure, including involvement of one or both incisal angles or the canine cusp tip. They are intended for cases where the defect cannot be managed by minor incisal edge repair and where there is a reasonable expectation of success. Veneers are not appropriate when periodontal health is uncontrolled, when severe bruxism exists, or when the remaining tooth structure (including lingual surfaces) cannot adequately support a veneer; in such circumstances a full crown or alternative treatment may be more appropriate.
