Coverage Criteria for Veneers

Medically necessary criteria for veneer restorations

Veneer restorations are considered when ALL of the following are met:

Primary coverage criteria: 1) Placement of veneers must be necessary, appropriate and meet generally accepted standards of care.

Primary coverage criteria: 2) Must have a reasonable expectation for success.

Primary coverage criteria: 3) A current (within 12 months), pretreatment periapical radiographic image showing the apex is required for evaluation of lack of pulpal and/or periapical pathosis.within 12 months

Placement of veneers is considered when the service is necessary, appropriate and meets generally accepted standards of dental practice. Indications include anterior teeth that have sustained significant loss of tooth structure such that conservative veneer restoration is required (for labial veneers, this includes involvement of one or both incisal angles or a canine cusp tip). Evaluation must show a reasonable expectation for success, and a current (within 12 months) pretreatment periapical radiograph showing the apex is required to assess pulpal and periapical status. Fully intact lingual surfaces must be present to support veneer placement when large areas of decay exist, and periodontal health and long-term prognosis must be documented prior to placement.

Providers must submit sufficient documentation to support medical necessity and plan review. Required documentation includes a current (within 12 months) pretreatment periapical radiograph showing the apex, clinical chart notes, reports, and photographic documentation when requested. Documentation of periodontal status and history of definitive periodontal treatment is required when applicable. Failure to provide requested radiographs, clinical notes, photographs, or other supporting items may delay review or result in denial.

Conservative-first restorative approach

Veneers should only be considered after conservative restorative options have been evaluated. When tooth structure loss is minor, less invasive alternatives (for example, direct or minimally invasive restorations) are preferred over veneers. Providers must document why conservative measures are not appropriate and show a reasonable expectation for success with veneering.

Denial Risk

Cosmetic or developmental exclusions

Veneer placement for purely cosmetic reasons, for correction of congenital or developmental defects, or to correct tooth size discrepancies is not covered. Placement of veneers in the presence of uncontrolled or untreated periodontal disease, or for patients with severe bruxism or grinding that would compromise prognosis, is not appropriate and will not meet contractual criteria. Benefits for replacement of defective veneers are subject to contractual frequency limitations.

Background

Veneers are a conservative restorative option intended to restore function when anterior teeth have sustained significant loss of tooth structure such as involvement of incisal angles or canine cusp tips. They are not appropriate for minor incisal edge/angle loss that can be managed with less invasive measures, nor are they suitable for patients with severe bruxism which compromises long-term prognosis. Documentation of the degree of tooth breakdown (reports, chart notes, photographs) and pretreatment radiographs are required to support clinical necessity.
