Guideline governing appropriateness and coverage considerations for local delivery of chemotherapeutic or antimicrobial agents as adjunctive therapy in periodontal disease management, including indications, contraindications, required documentation, timing relative to periodontal therapy, and applicable CDT procedure codes (informational).
Criteria #11 added: LDCA/LDAA adjunctive therapy will not be considered for benefit when performed on same date of service as scaling and root planing.
