Benefit limits and coding considerations: Benefits generally limited to two quadrants per date of service and generally limited to one periodontal surgical procedure in a 36/60 month period per single tooth or multiple teeth in the same quadrant. Gingivectomy to allow access for crowns/restorations is often incidental to the primary restorative procedure and group contract dependent. Laser or electrosurgery use is considered an adjunct/technique and is not eligible for additional separate benefit.

Gingivectomy and gingivoplasty are covered when documentation demonstrates treatment of mild to moderate periodontal disease after unsuccessful non‑surgical therapy and the procedure’s clinical intent is to remove loose or diseased gingival tissue, eliminate suprabony pockets, or re‑sculpt excess gingival tissue (including drug‑induced hyperplasia) to restore normal gingival architecture. The procedure must be planned and documented to reduce pocket depth and/or improve access for restorative care rather than to address infra‑bony disease.

Required documentation includes current (within 12 months) dated 6‑point periodontal charting showing pocket depth recordings and current (within 12 months) pretreatment radiographs that show the periapical area and an undistorted image of the alveolar crest. Charting must be performed after completion of non‑surgical periodontal therapy (e.g., scaling and root planing 4341/4342) and/or periodontal maintenance (D4910). Coverage limits generally apply: benefits are typically limited to two quadrants per date of service, and to one periodontal surgical procedure per single tooth or multiple teeth in the same quadrant in a 36/60‑month period, with group contract variations and case‑by‑case exceptions.

The following are excluded or considered contraindications to gingivectomy/gingivoplasty: treatment of infra‑bony pockets, pockets that extend below the mucogingival junction, procedures that require access to or removal of alveolar bone, and cases with minimal amounts of attached keratinized tissue. These scenarios are not appropriate indications for gingivectomy and may be denied.

Gingivectomy performed solely to correct altered passive eruption is not benefited. Also note that when diagnostics indicate adequate access based on periodontal support (bone level, gingival level, and/or recession), a gingivectomy may not be necessary and should be evaluated accordingly.

Gingivectomy performed within six months of orthodontic treatment is considered cosmetic and is generally not covered. The use of lasers or electrosurgery is considered an adjunctive technique and is not eligible for a separate additional benefit.

Allow a minimum healing period of four weeks after completion of non-surgical therapy to permit tissue healing and reassessment of pocket depths.

Contraindications — Infra‑bony Pockets and Procedures Requiring Access to Alveolar Bone

Gingivectomy/gingivoplasty is contraindicated for infra‑bony pockets, pockets extending below the mucogingival junction, or procedures that require access to the alveolar bone. Such cases require alternative surgical approaches (e.g., flap surgery with access to bone) and are not appropriate for gingivectomy/gingivoplasty billing or additional benefit.

Do not bill gingivectomy/gingivoplasty (D4210/D4211/D4212) when treating infra‑bony defects or pockets extending below the mucogingival junction.
Procedures that require access to alveolar bone (including anatomical crown exposure removing supporting bone) should be coded appropriately (e.g., D4230/D4231 or other applicable codes) and are not eligible for additional benefit as a gingivectomy adjunct.

Background

Gingivectomy and gingivoplasty are surgical procedures to remove diseased or excess gingival tissue, eliminate suprabony pockets, and restore normal gingival architecture. Clinically, they are intended to reduce pocket depth, re‑shape hypertrophic gingiva, and improve access for restorative or periodontal care rather than to treat infrabony defects.

These procedures are commonly used after non‑surgical therapy has failed to resolve pocketing or in cases of gingival enlargement (including drug‑induced hyperplasia). The use of lasers for gingivectomy is a technique choice and considered an adjunct; laser use does not warrant an additional separate benefit.
