Primary indications: Tooth has pulpal disease (inflamed or infected pulp) due to deep decay, repeated procedures, faulty crowns, a significant crack/fracture/chip, or trauma; or failure of initial endodontic therapy; or internal/external resorption with favorable prognosis.

Prognosis & restorative considerations: Crown-to-root ratios poorer than 1:1 create an unfavorable prognosis and must be assessed in the context of full mouth and periodontal status, medical/dental history, and occlusion. Placement of a restoration on a tooth with untreated or unresolved periapical/periradicular pathology, or a carious lesion close to the pulp without planned endodontic therapy, will not be considered for benefit.

Procedure scope: Pulp testing, cleaning, shaping, irrigation and related steps (including intraorifice barrier, temporary access closure, obturation, and treatment of root canal obstruction) are considered part of completed endodontic therapy.

Apicoectomy / Periradicular surgery: Apicoectomy or other periradicular surgery is allowed when periradicular pathology cannot be eliminated by non-surgical endodontic therapy; non-surgical retreatment should be attempted first. Apicoectomy is allowed once per root per tooth per lifetime and requires pre- and post-operative radiographs.once per root per tooth per lifetime

Pulpotomy and Apexogenesis

Covered when ALL of the following are met

Pulpotomy indications: Pulpotomy indicated for exposed vital pulp or irreversible pulpitis of primary (deciduous) teeth; pre-operative radiograph and post-operative radiograph are required. Pulpal debridement or pulpotomy may be used as an emergency or interim procedure in permanent teeth but benefits may be group dependent.

Apexogenesis indications: Apexogenesis is allowed once per tooth per lifetime when there is no evidence of periapical pathosis, the tooth has a deep carious lesion likely to expose the pulp during excavation, bleeding can be controlled at exposure, an adequate coronal seal can be maintained, and the patient is informed of the possibility of future endodontic therapy. Pre- and post-operative radiographs are required.once per tooth per lifetime

Root Resection

Covered when ALL of the following are met

Root Resection: Root resection is appropriate for periodontal furcation defects with infrabony defects; inoperable carious, resorptive, or iatrogenic root defects; vertical root fracture confined to the root to be separated in a multi-rooted tooth; or persistent periradicular pathosis where non-surgical therapy or periradicular surgery is not possible. Pre- and post-operative radiographs are required and adequate bony support and crown-root ratio around remaining roots must remain.

Endodontic therapy (root canal procedures) is covered only when there is evidence of pulpal disease (inflamed or infected pulp) or other clinical indications described in the policy and when documentation requirements are met. Documentation should include results of pulp testing when applicable and a recent, dated, and properly identified pre-treatment full mouth or panoramic radiograph that includes the radiographic apex (required within 12 months).

Treatment planning must consider overall prognosis and restorative factors; a crown-to-root ratio poorer than 1:1 is considered unfavorable and requires a full-mouth and periodontal assessment before proceeding. Placement of a restoration on a tooth with untreated or unresolved periapical or periradicular pathology, or on a tooth with a carious lesion in close proximity to the pulp without planned endodontic therapy, will not be considered for benefit.

Radiographic and clinical records (including detailed chart notes when cracked tooth syndrome is suspected) are required to support medical necessity. Providers should also note that many procedural elements such as pulpotomy, irrigation, obturation, and intraorifice barriers are considered part of completed endodontic therapy and are addressed within the procedure codes and coverage rules.

Placement of a definitive restoration on a tooth with documented internal or external resorption may not be considered for benefit unless the resorptive lesion has a favorable prognosis and documentation supports that restorative intervention is appropriate. The policy distinguishes between resorption with a favorable prognosis (which may be considered) and resorption with questionable or unfavorable prognosis (which is not appropriate for endodontic therapy or restorative coverage).

When resorption is present, pre-operative radiographs and clinical documentation must demonstrate the extent and prognosis of the lesion; treatment decisions should align with the policy criteria regarding resorptive lesions and overall tooth prognosis.

Procedures that are intended to treat asymptomatic teeth without pulpal disease or teeth that have an unfavorable prognosis are not appropriate under this policy. Examples include attempts at endodontic therapy when there is no documented pulpal pathology and cases with extensive caries involving the furcation, extensive alveolar bone loss from periodontal disease, or furcation involvement with extensive bone loss.

Before initiating endodontic procedures, providers must evaluate and document pulpal status, periodontal condition, and overall restorative prognosis. When prognosis is unfavorable as described in the policy (including significant resorption of questionable prognosis), such procedures will be considered not medically necessary.

Trauma-related fractured tooth claims: Claims for fractured teeth resulting from external blow or blunt trauma must first be submitted to the member's medical/health carrier when care is provided within the first 12 months post-injury. If the tooth is treated for a fracture, documentation must show missing tooth structure that extends into the dentinal layer.

Surgical precedence

Surgical precedence: When periapical pathology is present, non-surgical/conventional endodontic therapy (including retreatment when appropriate) should be attempted first prior to apicoectomy. Apicoectomy is considered when nonsurgical therapy cannot adequately eliminate periradicular pathosis or when access to additional roots or other anatomic factors necessitate surgery.

Background

Endodontic therapy is performed to treat inflamed or infected dental pulp resulting from causes such as deep caries, repeated dental procedures, faulty restorations, cracks or fractures, and trauma. Indications also include failed prior endodontic therapy and internal or external resorption when the prognosis is favorable.

The goal of therapy is to eliminate infection, prevent or resolve periradicular disease, and allow restoration of the tooth to function. Treatment decisions should follow generally accepted standards of dental practice and be supported by clinical findings and appropriate radiographic documentation.
