Local delivery is used as adjunctive therapy to treat refractory pockets following completion of definitive non-surgical and/or surgical periodontal treatment or in conjunction with periodontal maintenance.
Target site is a refractory/active periodontal site defined as a periodontal pocket measuring 5 millimeters or greater>= 5 mm
Site demonstrates persistent signs of inflammation, spontaneous bleeding or bleeding on probing, suppuration and/or increasing loss of clinical attachment

Not Appropriate / Exclusions

Not covered when ANY of the following apply:

Background & Evidence

Local delivery of chemotherapeutic/antimicrobial agents (LDCA/LDAA) may be used as an adjunct to scaling and root planing (SRP) or periodontal maintenance for isolated refractory periodontal sites. SRP is the standard non-surgical therapy and most sites respond without adjunctive local delivery.

Use for alveolar bone regeneration is considered experimental and investigational because LDCA/LDAA products have not been clinically tested for that application.

Evidence summary: Publish Date 01/01/2026. Key references include multiple peer-reviewed trials and AAP/ADA statements (e.g., J Periodontol 2000; J Periodontol 2006) and systematic reviews from 2003–2015 supporting adjunctive local delivery research and guidance.

Refractory disease site: Area of periodontal pocketing ≥ 5 mm with persistent signs of inflammation, spontaneous bleeding or bleeding on probing, suppuration and/or increasing loss of clinical attachment.

LDCA/LDAA: Local delivery of chemotherapeutic agents / local delivery of antimicrobial agents used as a subgingival, controlled-release adjunctive therapy for isolated refractory periodontal sites or during periodontal maintenance.

The plan considers use of LDCA/LDAA for alveolar bone regeneration experimental and investigational because these products have not been clinically tested for regeneration of alveolar bone. The policy explicitly excludes this application.

Additionally, Criteria #11 was added in the most recent reviews clarifying that LDCA/LDAA adjunctive therapy will not be considered for benefit when performed on the same date of service as scaling and root planing.

Denial Risk

Claims may be denied when treatment is performed on sites <5 mm, on same date as scaling and root planing, for generalized periodontitis requiring more extensive therapy, or for use in alveolar bone regeneration (experimental).
