Medical-Necessity Criteria

General Coverage Criteria

Covered when ALL of the following are met:

ALL of the following

Procedure restores function by replacement of missing teeth in a dental arch and retainer teeth exhibit periodontal and endodontic health and alignment to direct occlusal forces along long axis
Service meets generally accepted standards of dental practice (credible peer-reviewed evidence, specialty recommendations, practicing dentists' views, other credible sources)
Not primarily for convenience, not cosmetic, and not more costly than an alternative service
For plans with Missing Tooth Clause (MTC): no benefit for replacement of teeth missing prior to member's plan effective date (MTC does not apply to replacement prosthetics)

Specific Retainer Tooth Criteria (Anterior vs Posterior)

Retainer tooth considered on its own merit when ONE of the following applies

Anterior retainer: Significant loss of coronal tooth structure due to caries, fracture, or defective restoration and/or involvement of one or both incisal angles
Posterior retainer: Significant missing tooth structure due to caries, fracture, or defective restoration; large restorations that compromise function; or loss of support for the cusps where cusps are undermined (one or more cusps) with compromised mesial or distal marginal ridges

Trauma / Fracture Related Coverage

Covered when ALL of the following are met:

ALL of the following

Fractured tooth resulted from an external blow or blunt trauma (accidental traumatic injury)
Claims for fractured teeth from trauma should first be referred to the subscriber/employee's medical/health plan
Fracture involves missing tooth structure that extends into the dentinal layer
Claims for fractured teeth from trauma should first be referred to the subscriber/employee's medical/health plan

Periodontal Status Requirement

Benefits are NOT considered when ANY of the following apply:

Submit current (within 12 months), dated diagnostic pretreatment radiographic images that include the radiographic apex. Include clinical chart notes, intra‑oral photographs, and a current dated 6‑point periodontal chart. Provide adjunctive intraoral photos and a rationale when radiographs are insufficient.

Denial Risk

Missing Tooth Clause denials

For plans that include a Missing Tooth Clause (MTC), the pontic portion of a fixed partial denture will deny if the tooth was missing prior to coverage. Retainers will still be reviewed on their own merit.

An indirect restoration placed on a tooth with untreated or unresolved periapical or periradicular pathology will not be considered for benefit and may be denied.

Billing Rule

Delivery date definition

The delivery date of an indirect restoration is the date of initial cementation or bonding regardless of cement type. The date of service/payment may be contract dependent.
