Evaluate statin therapy when ANY of the following guideline-based scenarios apply:

Persons with diabetes aged 40–75: support use of moderate-intensity statin therapy to reduce ASCVD risk.age 40-75 with diabetes

Recommendation regardless of LDL values.

Patients with clinical ASCVD (males 21–75 and females 40–75 as cited) should receive high-intensity statin therapy unless contraindicated, particularly when the estimated 10-year ASCVD risk is >= 7.5%.clinical ASCVD and age ranges specified

Document the clinical assessment that supports your decision regarding statin therapy. Include: • Assessment of ASCVD risk and indication for therapy (e.g., diabetes, clinical ASCVD). • Rationale for initiating or not initiating a statin and chosen statin agent and dose (moderate- vs high-intensity). • Relevant patient factors (age, contraindications, intolerance, prior adverse reactions, current lipid values if available). Proper documentation supports medical necessity and care continuity.

Evaluate Medicare Advantage members with diabetes and/or cardiovascular disease for statin therapy per current ACC/AHA guidance. For patients 40–75 years with diabetes, consider at least moderate‑intensity statin therapy; consider high‑intensity therapy for patients with clinical ASCVD or elevated 10‑year ASCVD risk as recommended by guideline thresholds. Failure to evaluate and document consideration of statin therapy for eligible MA members may conflict with CMS quality expectations.
