Coverage criteria for unbundled services

The plan will deny separate reimbursement for component procedure codes when national unbundling edits or the plan's code editing software determine the component is not separately reimbursable. Certain modifiers and supporting documentation can justify separate payment.

Deny component procedure code when billed on the same date with a more comprehensive procedure; the code editing program will recommend denial if a more comprehensive procedure is billed.

Deny when procedures result in overlapping services or are considered impossible to perform together during the same operative session; the code editing program will recommend denial in these cases.

Deny when an E/M is billed on the same date as a surgical procedure unless an approved exception applies; the code editing program will recommend denial if the E/M is not separately supported.

Exceptions — reimburse E/M or component service when documentation supports separate payment

Modifier 25 appropriately appended and documentation demonstrates a significant, separately identifiable E/M that goes above and beyond the other service (see criteria list).
Evidence that Modifier 25 is appropriate, such as: the E/M is the first time the provider has seen the patient or evaluated a major condition; a diagnosis on the claim shows a separate medical condition was treated in addition to the procedure; the patient’s condition is worsening as evidenced by diagnostic procedures on or around the date of service; unrelated supplies/equipment billed that required an E/M to determine need.

Modifier 57 exception: Modifier 57 may justify separate reimbursement for an E/M when the decision to perform major surgery (90-day global) was made the day before or the day of surgery; such claim lines are subject to prepayment clinical claims validation and the E/M is paid only if documentation supports the initial decision to perform surgery.

Modifier 59 exception: Modifier 59 may justify separate payment for non‑E/M services/procedures not typically reported together when documentation and diagnosis codes show distinct treatment, different site/session/organ system, or other reasons (e.g., second initial injection at a separate site); all applicable anatomical modifiers and diagnoses should be present to avoid incorrect denials.

Additional modifier review: Prior to denial, the plan considers Modifier 51 and laterality‑specific modifiers (for example RT/LT) in its determination; these modifiers may affect whether component codes are denied.

Providers should bill with awareness that same‑day professional services by the same provider are subject to the plan's unbundling edits and automated code editing software.
