Coverage Criteria — Qualifying Clinical Trials

Medical necessity criteria for routine clinical trial costs

Covered when ALL of the following are met

I. General policy: Routine costs of a qualifying clinical trial and services to diagnose/treat complications from participation are medically necessary per guidelines and limitations.

Policy applies to items/services generally considered medically necessary and covered benefits when provided in either arm of the trial.

II. What constitutes routine costs: Includes: (1) items/services typically provided absent a clinical trial; (2) items/services required solely for provision of the investigational item/service, clinically appropriate monitoring, or prevention of complications; (3) items/services needed for reasonable/necessary care arising from the investigational item/service, particularly diagnosis/treatment of complications.

III. Exclusions: Not covered: (1) investigational item/service itself; (2) items/services solely for data collection/analysis not used in direct clinical management (e.g., monthly CT scans when normally a single scan); (3) items/services customarily provided free by research sponsors.

Providers must obtain prior authorization and submit required notifications per Ambetter Nevada policies before performing services related to routine care for trial participants.

Required Trial Documentation

Providers must maintain and submit complete trial documentation to support coverage of routine costs. At minimum include the trial protocol, documentation of IRB approval, and evidence the trial meets qualifying criteria (trial purpose, funding/approval source, therapeutic intent, and that the intervention falls within a covered benefit category). Failure to provide required documentation may result in claim denials or coverage delays.

Therapeutic intent & populationThe trial must have therapeutic intent (not solely testing toxicity/pathophysiology) and therapeutic intervention trials must enroll patients with the diagnosed disease.

Acceptable funding/approval sourcesTrials must be federally funded or approved by one of the listed entities (AHRQ, CDC, CMS, NIH, cooperative groups, qualified nongovernmental entities, VA/DoD/DoE peer-reviewed approvals, FDA IND-reviewed studies, or exempt drug trials).
