Policy governs coverage of outpatient specialized therapy evaluations and treatments (physical, occupational, speech-language, respiratory, audiology) for NC Medicaid beneficiaries receiving services outside inpatient hospital/rehab settings.
Amended visit limitations for beneficiaries aged 21 and older (effective December 1, 2023) and reorganized subsections including definitions and ordering practitioner requirements.
Updated annual maximum treatment visit counts to a total maximum of 30 treatment visits per calendar year for specified therapy categories.
Removed prior requirement that the first PA request be three visits and one month and clarified PA request limits (single PA can request up to 12 therapy treatment visits and six months).
Added comprehensive lists of covered CPT codes for audiology evaluation and treatment in Attachment A.
