Defines Medicaid coverage, eligibility, and medical necessity criteria for rhinoplasty, septoplasty, and septorhinoplasty for NC Medicaid beneficiaries, including EPSDT exceptions for beneficiaries under 21.
Created a new subsection specifically for septoplasty criteria and added/clarified multiple septoplasty indications (including recurrent sinusitis defined as four or more episodes in a year, recurrent epistaxis of four or more significant episodes, performed with cleft repairs).
Added documentation requirements including number of sinus infections in last 365 days, number of epistaxis episodes when applicable, and postoperative plan of care.
Removed the table of CPT codes that do not require prior approval.
