Coverage Criteria for Wireless Capsule Endoscopy

Specific Criteria Covered

Covered when ALL of the following are met for each applicable indication as specified below

3.2.1.a Undiagnosed obscure gastrointestinal bleeding: ALL: 1) GI bleeding is significant as demonstrated by one of the following: (A) an acute drop in hemoglobin/hematocrit; (B) unexplained recurrent or persistent iron deficiency anemia demonstrated by low serum iron studies or low serum ferritin level; (C) persistently positive fecal occult blood test; or (D) visible bleeding with no bleeding source found at original endoscopy; 2) Failure of previous diagnostic studies to diagnose the source of GI bleeding, including upper and lower GI endoscopy (EGD or colonoscopy) within the past 12 months; 3) Source of GI bleeding is thought to be in the upper gastrointestinal tract.

3.2.1.b-c Suspected esophageal conditions: Covered for suspected Esophageal Varices OR suspected Barrett's Esophagus.

3.2.1.d Suspected Crohn's Disease: ALL: 1) Persistent abdominal pain of greater than 4 weeks; 2) Persistent diarrhea with one or more signs of inflammation (fever, elevated white blood cell count, elevated erythrocyte sedimentation rate, or bleeding); 3) Unintentional weight loss; 4) Negative stool cultures; and 5) Negative upper and lower endoscopy studies.

3.2.1.e-f Celiac disease and hereditary polyposis surveillance: Covered for (e) suspected Celiac disease with a positive serology and negative biopsy; and (f) surveillance of the small intestine for beneficiaries with hereditary polyposis syndromes.

Device requirement: The wireless capsule must be approved by the Federal Drug Administration (FDA).

Coverage contingent on prior approval

Covered only when prior approval is obtained and Subsection 3.2 criteria are met.

Prior approval and documentation: Prior approval is obtained before rendering Wireless Capsule Endoscopy and the provider submits the prior approval request plus all health records and supporting documentation to the DHHS Utilization Review Contractor demonstrating the beneficiary meets the specific criteria in Subsection 3.2.

Exception: Prior approval is not required when the procedure is initiated in the emergency department for a potentially life‑threatening condition, provided the Subsection 3.2.1 criteria are met; in such cases the electronic PA and supporting documentation must be submitted per policy timelines.

Medicaid shall not cover capsule system testing or radiography to evaluate gastrointestinal patency prior to wireless capsule endoscopy. Medicaid shall not cover wireless capsule endoscopy in the following situations: undiagnosed obscure GI bleeding; known or suspected gastrointestinal obstruction, stricture, fistulae, or known bowel disease; recurrent intussusception; duodenal lymphocytosis; surveillance of the small intestine for hereditary polyposis syndrome; as a first‑line diagnostic tool for diffuse abdominal pain; as a replacement for colonoscopy for colon cancer screening per USPSTF recommendations; or when the specific coverage criteria in Subsection 3.2 are not met.

Wireless capsule endoscopy is not covered for the listed specific indications including: undiagnosed obscure gastrointestinal bleeding; known or suspected GI obstruction, stricture, fistulae, or known bowel disease; and for use in the evaluation or diagnosis of suspected conditions such as Crohn's disease, irritable bowel syndrome (IBS), esophageal varices, Barrett's esophagus, celiac disease, gastric or intestinal neoplasm, or intestinal polyps.

In addition to the specific non‑coverage list, Medicaid shall not cover wireless capsule endoscopy when the beneficiary is pregnant or when the beneficiary has an implanted electromagnetic device such as a cardiac pacemaker, defibrillator, spinal cord stimulator, or other similar device. The policy also notes that the wireless capsule must be approved by the Federal Drug Administration (FDA).

Verify Medicaid eligibility for each beneficiary at the time of service. For beneficiaries under 21 years of age, document EPSDT medical necessity in the record when the service is being requested or provided. EPSDT documentation must show how the requested service is medically necessary to correct or ameliorate a defect, physical or mental illness, or a condition identified through screening, and may justify exceptions to policy limits when supported by the medical record.

Background and Scope

Wireless capsule endoscopy involves the beneficiary swallowing a small, self‑contained capsule that contains a camera and transmitter. The capsule transmits images to an external recorder over several hours for subsequent physician download and interpretation, allowing visualization of the esophagus and small intestine mucosa as the capsule traverses the GI tract and is naturally excreted.
