Qualifying conditions for beneficiaries under 21: Presence of any one of the following: inappropriate growth or weight gain (including inadequate weight gain, inappropriate weight loss, underweight, obesity, inadequate linear growth, or short stature); nutritional anemia; eating/feeding disorders resulting in medical conditions (e.g., failure to thrive, anorexia nervosa, bulimia nervosa); physical conditions impacting growth/feeding (e.g., very low birth weight, necrotizing enterocolitis, cleft palate, cerebral palsy, neural tube defects); chronic or prolonged infections with nutritional component (e.g., HIV, hepatitis); genetic conditions affecting growth/feeding (e.g., cystic fibrosis, Prader-Willi, Down Syndrome); chronic medical conditions (e.g., cancer, cardiac disease, hypertension, hyperlipidemia, gastrointestinal disease, liver disease, pulmonary disease, malabsorption syndromes, renal disease, significant food allergies, immune diseases); metabolic or endocrine disorders (including inborn errors of metabolism such as PKU; diabetes); non-healing wounds due to chronic conditions; acute burns over significant body surface area; metabolic syndrome; or documented first-degree relative with cardiovascular disease or significant cardiovascular risk factors.at least one condition present
