Adult escort coverage with medical necessity: Escort may be covered for adults (18+) when a functional limitation supports medical necessity (examples: immobility, significant cognitive or physical dysfunction, blindness, undergoing cancer therapy, sedated procedures, or an anxiety disorder being treated by a behavioral health provider); typically requires a current Travel COMN (Section 2) or provider certification on file.

Examples of noncoverage for escorts: Escort is not covered for adult members needing an escort solely for translation (limited English) or for developmentally disabled adults without significant cognitive or physical dysfunction; mobile elderly able to ambulate with a cane or walker are not covered for an escort.

Travel benefits are available only when the member has no personal means of transport to access the needed healthcare services and no voluntary services are available. Requests may be denied when the member has personal transportation or when voluntary options exist. When arranging travel, coordinators must attempt to combine appointments into a single trip when possible to reduce unnecessary travel and protect program integrity.

Out‑of‑area travel is explicitly excluded when the service can be delivered via telehealth or will be available locally within 3 months, unless a licensed provider documents that earlier in‑person care is required and telehealth is not feasible. For out‑of‑area therapies and higher‑level services, a Travel COMN establishing medical necessity and other required documentation must be on file.

Escort coverage is limited by functional need: escorts are not covered for adult members who require assistance only for translation or who are mobile and able to ambulate with a cane or walker. Escorts for adults may be covered when a functional limitation supporting medical necessity is documented and an appropriate Travel COMN (Section 2) or certification is on file.

Initial visits for diagnostic, therapeutic, or treatment services are not covered when the member does not have a referral from a licensed provider; subsequent visits are eligible once medical necessity is established and documentation (e.g., POC) is on file.

Travel exclusively for vaccine administration — i.e., travel outside a member’s home community solely to receive vaccinations — is not covered for both children and adults.

Travel requests will be denied when the service is expected to be available locally within 3 months or telehealth is a feasible option, unless a licensed provider documents the need for earlier in‑person care and telehealth is not feasible. Preventive out‑of‑area travel, when covered, is limited to one per calendar year and must be combined with other travel when possible.
