A published meta-analysis of serum vitamin D and kidney stone risk pooled data from 7 studies (451 cases, 482 controls) and reported higher serum vitamin D levels in stone formers; however, the analysis has important limitations. Most included studies (6 of 7) were case‑control in design, potential confounders may not have been adequately controlled, geographic representation was limited (results derive mainly from Europe and Asia with only one U.S. study), and between‑study heterogeneity was high and unexplained. These factors limit applicability of the findings to broader populations and to causal inference.

Untargeted urine metabolomics studies have identified metabolites that are enriched in stones and in the urine of active stone formers, and a combined 14‑metabolite model classified active versus non‑active stone formers with ~73% accuracy in a prospectively recruited cohort. However, the investigators note limitations—including potential effects of renal function, medications, differences in imaging modalities, and lack of stratification by stone composition—and explicitly state that the identified urinary metabolites are candidate lithogenic agents; a causal role in stone formation has not been established and further studies are required to confirm these findings and define mechanisms.
