Medical Necessity Criteria for ABA: 1) DSM‑V diagnosis of Autism Spectrum Disorder (ICD‑10: F84.0; F84.3‑F84.9) obtained by an appropriate provider; 2) Services provided directly or billed by an appropriately licensed provider; 3) Demonstration of functional impairment on a standardized scale within the past 12 months (e.g., VABS‑3, ABAS, VB‑MAPP, ABLLS) with impairment at least 1 standard deviation below the population mean or representing significant risk of harm to self/others; 4) An individualized, time‑limited treatment plan documenting specific target behaviors with baseline measures and quantifiable progress criteria, behavioral intervention techniques, selected reinforcers, generalization strategies, transition/titration and discharge criteria, and a gradual tapering plan as progress occurs; 5) Documentation of improvement over the course of treatment OR if no improvement, documentation of treatment modification, additional assessments, and/or appropriate consultations; 6) The level of impairment justifies the number of hours requested.

Severity-to-hours mapping (guide): For each domain (maladaptive behavior, social communication, self-care): None (<1 SD below mean) = 0 hrs/week; Mild (>1 SD below) = 0.1–4 hrs/week; Moderate (>1.5 SD below) = 4–7 hrs/week; Severe (>2 SD below) = 7–10 hrs/week. Authorized hours are based on documented severity across these domains.

Additional authorizations: Based on functional impairment and assessment of symptom severity, additional authorization may be provided for QHP protocol modification and direction at 1–2 hours per 10 hours of treatment, and authorization for caregiver training.

When coverage terms in this guideline conflict with a member's benefit plan or a state or federal mandate (including CMS rules for Medicare and Medicaid), the member's benefit plan or applicable law governs. Plans or government programs may impose different coverage limits, requirements, or procedures that take precedence over the criteria in this document.

Services that are primarily custodial in nature—where the main purpose is assisting with activities of daily living without reasonable expectation of significant clinical improvement—do not meet medical necessity for ABA. Aetna defines custodial care as care provided when a member has reached maximum physical or mental function and is unlikely to make further significant improvement, or care whose primary purpose is attending to daily living activities that do not require continuing attention of trained medical personnel. Plan documents may vary; review the member's plan for any differing definitions or limits.

Prior Authorization Required

Prior authorization required for applied behavior analysis (ABA) services except when provided in an inpatient, residential, or partial hospitalization (PHP) setting. If ABA is delivered in one of those higher levels of care, applicable medical necessity for that level of care is used and a separate authorization for ABA is not required; however, prior to discharge a review using the ABA medical necessity criteria is required.

Prior authorization required for outpatient/community ABA services.
No separate ABA prior authorization required when ABA is provided in inpatient, residential, or PHP settings; review ABA medical necessity prior to discharge.

Documentation Required

Maryland Procedural Requirements (Exhibit A)

For members covered under Maryland plans, Aetna will apply the Maryland-specific procedural requirements in COMAR 31.10.39 when reviewing medical necessity for ABA.

The child’s primary care provider (PCP) or specialty physician must perform a comprehensive evaluation identifying the need for ABA and must prescribe the treatment with specific treatment goals.
Treatment must be reviewed annually for medical necessity by the PCP or specialty physician in consultation with the ABA provider and include documentation of benefit, identification of new or continuing goals, and development of a new or continuing treatment plan.
ABA provider must be licensed/certified/authorized under Maryland law (or equivalent elsewhere).
Coverage may still be subject to plan limits (COB, participating provider rules, restrictions on family-provided services, case management, cost sharing).
Aetna will not deny coverage solely because of the number of ABA hours prescribed, the location of services being an educational setting, or on the basis that ABA is experimental/investigational.

Note

Requesting Auxiliary or Language Services

Providers must request auxiliary aids, reasonable modifications, or language assistance through Aetna’s established contact points. Failure to request or arrange needed auxiliary or language services when required may expose the member to communication barriers and could result in grievances or civil rights complaints.

To request reasonable modifications, auxiliary aids and services (e.g., qualified sign language interpreters, large print, audio, accessible electronic formats) or language assistance (e.g., qualified interpreters, information in other languages) call 1-800-872-3862 (TTY: 711) or the number on the back of the member’s ID card.

Outpatient and Higher Level Care Guidance

Inpatient/Residential/PHP ABA: When ABA is provided in an inpatient, residential, or partial hospitalization setting, the medical necessity criteria applicable to that level of care apply and separate authorization for ABA is not required; reviews occur at a frequency commensurate with the level of care. Prior to discharge from a higher level of care, a review using the outpatient ABA medical necessity guideline is needed.

Continued ABA authorization: Documented progress should be evaluated at intervals (evaluate every 6 months) with ongoing justification that the level of impairment warrants continued hours; if no progress, transition to other services or document treatment modifications, additional assessments, or consultations.
