RFVTR medical necessity: Radiofrequency volumetric tissue reduction (RFVTR, Somnoplasty) is considered medically necessary for treatment of chronic nasal obstruction due to mucosal hypertrophy of the inferior turbinates.

Billing should use covered CPT codes 30801 or 30802 with supporting ICD-10 diagnosis codes for turbinate hypertrophy; conservative medical therapy (eg, intranasal steroids) is expected to have been attempted prior to surgical intervention.

When a procedure is performed for an indication that the policy explicitly lists as not covered, billing should not rely on covered CPT codes for RFVTR. The policy lists CPT 31242 and other technique-specific approaches as not covered for the indicated diagnoses and procedures; using covered RFVTR codes (30801, 30802) to report services performed for those excluded indications (for example, posterior nasal nerve ablation, treatment of obstructive sleep apnea, snoring, allergic rhinitis, or chronic rhinitis) may result in claim denial.

Additionally, some devices and techniques described in the policy have no specific CPT/HCPCS code (for example, temperature-controlled radiofrequency neurolysis and the NEUROMARK System). Those procedures should not be recoded to covered RFVTR CPTs when the clinical indication is one the policy identifies as not covered; instead, follow payer guidance for reporting unlisted procedures or device-specific reporting.

Placebo-controlled data demonstrate a substantial placebo response in inferior turbinate procedures. In the Harju et al randomized, single-blinded, placebo-controlled trial, symptom severity measured by VAS decreased significantly in the placebo group and all active-treatment groups, and although radiofrequency ablation produced additional statistically significant reduction versus placebo, the authors emphasized a large placebo effect. This trial’s findings should be considered when assessing incremental effectiveness for indications where sham responses are prominent.

Several trials excluded patients considered by investigators to have a predisposition to poor wound healing; therefore, applicability of the trial results to that subgroup is uncertain. The temperature-controlled RF studies note this exclusion explicitly and state that durability and safety in patients with poor wound healing were not evaluated.

Other limitations relevant to generalizability include lack of required allergy testing and uncontrolled medication use in some trials, which limit ability to stratify effectiveness by rhinitis subtype or to isolate the device effect from concomitant therapies.

Aetna considers RFVTR of turbinates for snoring to be not medically necessary. The policy distinguishes treatment of snoring from treatment of chronic nasal obstruction due to inferior turbinate mucosal hypertrophy, for which RFVTR may be medically necessary when selection criteria are met.

Procedures that show only placebo-level benefit in controlled trials may be considered not medically necessary for the assessed indication. The Harju et al placebo-controlled study demonstrated large placebo-associated improvements and raised uncertainty about the clinical significance of additional benefits from some turbinate procedures; this evidence limitation supports cautious consideration of interventions whose effects do not clearly exceed sham.

Older surgical procedures such as vidian nerve resection and other historical neurectomy techniques have not demonstrated sustained long-term benefit and carry potential risks (for example, persistent pain). UpToDate and the cited reviews note absence of long-term benefit for some of these older procedures, which is a rationale for considering them not recommended in current practice.

Background

Inferior turbinate hypertrophy is a common contributor to chronic nasal obstruction and may be managed initially with medical therapy (for example, intranasal corticosteroids and other topical agents). When mucosal hypertrophy is persistent despite conservative therapy, surgical reduction techniques are considered.

Radiofrequency volumetric tissue reduction (RFVTR, also called Somnoplasty) uses a submucosal radiofrequency catheter to create coagulative lesions that reduce submucosal tissue volume and turbinate size; it can be performed under local anesthesia in an office setting. Aetna considers RFVTR medically necessary for chronic nasal obstruction due to mucosal hypertrophy of the inferior turbinates when selection criteria are met, and directs use of covered CPTs 30801 and 30802 where appropriate.

Evidence for intranasal RF and related techniques consists largely of small prospective series and a limited number of randomized or controlled trials with variable follow-up.
