Initial orthotic banding for positional plagiocephaly

Positional plagiocephaly banding criteria: Banding initiated between 3 and 12 months of age; AND a 2-month trial of conservative therapy (repositioning) has failed and is judged unlikely to improve the deformity; AND one of the following: anthropometric data documents moderate-to-severe plagiocephaly (asymmetry > 6 mm) OR for brachycephaly cephalic index is >= 2 standard deviations above or below the mean for age/sex.

EPO and blood management

Perioperative blood-conservation strategies

Pre-operative EPO and blood conservation: Multiple studies including randomized and non-randomized series and a meta-analysis (4 studies, 117 patients) showed pre-operative erythropoietin (with iron supplementation and often intraoperative cell-salvage) increased pre-op hemoglobin/hematocrit and reduced the proportion of patients requiring allogeneic transfusion and transfused volumes; combined protocols were associated with fewer transfusions and shorter length of stay in some series.

Cranial remodeling bands or helmets are contraindicated and considered not medically necessary in infants with unshunted or uncontrolled hydrocephalus. Requests for devices in this setting will be denied. Calcified cephalohematoma is also a contraindication to band/helmet use. These exclusions align with the policy’s listed ICD-10 contraindication codes (for example, G91.* series and P12.0) and reflect concern that helmets do not address the underlying pathology and may be inappropriate or unsafe in these conditions.

Randomized trial evidence found no difference in skull-shape improvement at 24 months between helmet therapy and the natural course for otherwise healthy infants with moderate to severe positional skull deformation, and helmet use was associated with frequent side effects and high cost. For this reason, the policy discourages routine use of helmet (molding) therapy as a standard treatment for healthy infants and requires justification (objective severity measures and failed conservative care) when helmet therapy is requested.

Most cephalohematomas are managed expectantly because the clotted blood is gradually reabsorbed; aspiration after clotting is generally ineffective and increases the risk of infection and abscess. Early needle aspiration has been reported in a limited retrospective series for large lesions during a narrow window (around days 15–30) to avoid calcification, but routine aspiration is not recommended. Rare, large calcified or ossified cephalohematomas may require surgical excision.

For otherwise healthy infants with positional plagiocephaly or brachycephaly, helmet therapy is not routine first-line care. The policy requires a documented failed trial of conservative therapy (typically a 2-month trial of repositioning/physical therapy) and objective anthropometric evidence (for example, asymmetry ≥ 6 mm or cephalic index meeting the specified abnormal threshold) before cranial remolding orthosis is considered. Given randomized trial findings showing no superiority of helmets over the natural course for moderate–severe cases, requests for helmet therapy must be justified by severity, age, and failure of conservative measures.

Current evidence for the use of polycaprolactone (PCL) mesh as a sole adjunct to promote bone regeneration in pediatric cranial vault reconstruction is limited. Small case-series reported persistent bony gaps in areas with PCL mesh and, in at least one comparison, less bone regeneration where PCL mesh alone was used versus particulate bone graft. Therefore, PCL mesh used by itself lacks sufficient evidence of efficacy for routine reconstruction and is considered experimental/investigational in this context.

Background

Positional plagiocephaly most commonly results from infants spending extended periods in supine positions and is frequently responsive to conservative measures such as repositioning and age-appropriate motor development. True craniosynostosis (fusion of sutures) is uncommon and typically requires surgical correction. Cranial orthoses are custom-fitted devices intended to redirect skull growth over months of wear; when used for positional deformity they are generally initiated within an early age window to improve likelihood of correction and are contraindicated beyond 24 months. Given the variable natural history and evidence from randomized trials, conservative management is recommended first for most infants, with orthotic therapy reserved for objectively measured, persistent, or severe deformities after failed conservative care.
