Initiation criteria: Patient has active systemic lupus erythematosus (SLE) and does not have severe active central nervous system (CNS) lupus or severe active lupus nephritis (these conditions are specifically queried on the form).

Autoantibody and baseline therapy: Prior to initiating therapy, patient is positive for autoantibodies relevant to SLE when available (e.g., ANA, anti-dsDNA, anti-Sm, antiphospholipid antibodies, complement proteins) AND is currently receiving a stable standard treatment regimen for SLE (glucocorticoids, antimalarials, and/or immunosuppressives) unless clinically inappropriate.

Continuation Therapy

Covered when ALL of the following are met

Continuation criteria: Patient has achieved or maintained a positive clinical response as evidenced by low disease activity or improvement in signs and symptoms; provider attests to clinical response on the form.

Form includes checkbox asking whether patient has achieved or maintained a positive clinical response for continuation requests.

Ongoing appropriateness: There is documentation that the patient remains on an appropriate standard SLE treatment regimen as indicated and that continued use of the requested agent is medically appropriate given the patient’s current disease status and prior therapies.

Continuation requests should include clinical documentation consistent with initiation requirements and ongoing management.

Administration setting considerations

Consider administration setting when ANY of the following apply

Outpatient hospital administration: Infusion request is in an outpatient hospital setting.

Form asks whether infusion is in an outpatient hospital setting.

Serious or refractory infusion-related adverse events: Patient has experienced an adverse event with the requested product that has not responded to conventional interventions (e.g., acetaminophen, steroids, diphenhydramine, fluids, other pre-medications) or a severe adverse event (anaphylaxis, anaphylactoid reactions, myocardial infarction, thromboembolism, or seizures) during or immediately after an infusion.

Presence of serious or refractory infusion reactions may necessitate a higher-acuity setting.

Venous access issues: Patient has severe venous access issues that require the use of special interventions only available in the outpatient hospital setting.

Severe venous access problems are specifically queried on the form.

Behavioral/functional impairment without caregiver: Patient has significant behavioral issues and/or physical or cognitive impairment that would impact the safety of infusion therapy AND the patient does not have access to a caregiver.

Form requests description of behavioral or cognitive impairments that affect infusion safety.

Medical instability: Patient is medically unstable (including respiratory, cardiovascular, or renal conditions) that may limit tolerability of infusion volume or predispose to severe adverse events that cannot be managed in an alternate setting without appropriate medical personnel and equipment.

Form asks providers to indicate and describe cardiovascular, respiratory, renal, or other conditions relevant to setting selection.

Requests may be incomplete or may not meet medical necessity if they lack documentation of active systemic lupus erythematosus (SLE), documentation of relevant autoantibody positivity (e.g., ANA, anti-dsDNA, anti‑Sm, antiphospholipid antibodies, complement proteins) when available, or evidence that the patient is on a stable standard SLE treatment regimen (glucocorticoids, antimalarials, or immunosuppressives) unless clinically inappropriate. For continuation requests, the form requires documentation that the patient has achieved or maintained a positive clinical response (low disease activity or improvement in signs and symptoms) as attested by the provider.

Required Provider Actions and Documentation

Submit the two-page Saphnelo (anifrolumab-fnia) Injectable Medication Precertification Request form.

Documentation Required

Include primary and any secondary ICD-10 diagnosis codes.
Complete required clinical information: indication (active SLE), prior adverse events, venous access or behavioral/medical stability considerations, and for continuation requests document clinical response (improvement or low disease activity).
Attach supporting documentation of laboratory results (autoantibodies), and records demonstrating stable prior therapies where applicable.
Sign and date the acknowledgement on the form.
